FAQ
Here are answers to common questions from referral partners, families, and prospective clients. If you don't see your question here, contact us and we'll be glad to help.
Phoenix Place serves adults age 21 and older who are experiencing substance use disorders, mental health conditions, and/or re-entry from jail or prison. Most of our clients are referred by hospitals, jails, treatment programs, or community agencies. Clients must be able to live in a group setting and participate in programming. We primarily serve individuals with Kentucky Medicaid and can help connect uninsured clients to coverage when possible.
Referrals typically come from case managers, social workers, probation/parole officers, or other professionals. After an initial phone call to our office, we complete a pre-screen, verify insurance, and determine program fit. If accepted, we provide a bed letter when needed, schedule intake within 48 hours when possible, and complete a full clinical assessment and orientation on arrival.
Residents receive a combination of transitional housing, behavioral health services, peer support, and case management. This may include individual and group therapy, intensive outpatient programming (IOP), life skills coaching, and support with benefits, employment, and long-term housing. Each client has an individualized treatment plan with clear goals, responsibilities, and discharge criteria.
The average length of stay is around six months, though it varies based on clinical need, progress toward goals, and housing options. We complete regular reviews of each client's progress and update treatment and discharge plans accordingly. Our goal is always safe, sustainable transition to the next level of care or independent living.
